Submit your requests, and download medical records all in one place! Enter your email and we will send a secure link that lets you sign in.
You will then receive an email to confirm your registration and allow you to log your request.
Once submitted a notification is sent to our team to start processing your request. Once completed, you will be able to go online and download your records. Should you have any questions, please feel free to contact us at:
